GAINESVILLE, Texas – North Texas Medical Center (NTMC) has announced the launch of its new Chronic Care Coordination (CCC) Program, a proactive healthcare initiative designed to support residents living with multiple chronic conditions. This program marks a significant step in the hospital’s plan to address the evolving health needs of the Gainesville and Cooke County communities.
As identified in the recently approved 2026 Community Health Needs Assessment, chronic conditions such as diabetes, hypertension, and heart disease remain a primary concern for local residents. The CCC program at NTMC provides a structured, personalized approach to managing these long-term illnesses, moving beyond traditional office visits to provide a continuous “safety net” of support.
As part of the commitment to Chronic Care Coordination, NTMC has started a pharmacist-led Continuous Glucose Monitor (CGM) program for patients with diabetes. Many diabetic patients already utilize a CGM to manage their disease, but the numbers can be confusing. With this new program, an NTMC clinical pharmacist will work with patients and their doctors to turn that data into a clear plan for better health.
“With the launch of our new Chronic Care Coordination and Pharmacist-Led Continuous Glucose Monitoring (CGM) programs, North Texas Medical Center is fundamentally changing how we support our patients between office visits,” said NTMC Chief Clinical Officer Bonnie Hess. “By utilizing our clinical pharmacists and nurses as coaches, we are turning complex, real-time glucose readings into clear, actionable plans that empower patients to manage their health with confidence from the comfort of their own homes. This initiative isn’t just about managing illnesses; it’s about providing a proactive safety net that catches small health shifts before they turn into emergencies.”
The Chronic Care Coordination Program is available to eligible Medicare beneficiaries with two or more chronic conditions. Patients are paired with a professional coordinator who conducts check-ins to monitor symptoms and progress. The creation of a personalized electronic care plan will be accessible to the patient’s entire healthcare team. NTMC will also offer assistance with prescription adherence and identifying potential drug interactions. All of this will help the team identify health “red flags” to prevent acute exacerbations and hospital readmissions.
Patients interested in the program or providers looking to refer a patient are encouraged to contact NTMC Care Coordination Nurse Rina Pinnick at (940) 612-8642 or by email at chroniccare@ntmconline.net.
